要旨　胃MALTリンパ腫におけるStage Ⅰ除菌非奏効例は，おおむね症例全体の10～20％を占める．それらの中心をなすのは，t(11;18)染色体転座およびAPI2-MALT1キメラ遺伝子異常を有する症例である．これらは，ときにHelicobacter pylori感染が証明されず，肉眼的には粘膜下腫瘍様隆起，敷石状粘膜，浮腫状粘膜，あばた状粘膜を形成することにより特徴づけられる．病変の主座は，主に粘膜の下部，固有腺部から粘膜下組織である．粘膜表層には病変が及びにくく，リンパ上皮性病変の形成は不明瞭であることが多い．組織学的には反応性要素に乏しく，比較的単調な中間型腫瘍細胞の増殖を認める．t(11;18)(q21;q21)染色体転座は，胃リンパ増殖性病変の本態を理解するために非常に重要である．臨床的にも除菌療法に対する非奏効例を予測する上で極めて重要な分子指標であると考えられる．

　Gastric MALT lymphoma, which was unresponsive to H. pylori eradication, occupied 10～20％ of the cases, including H. pylori-positive and -negative ones. However, they shared such features as follows: polypoid gross appearance or cobblestone appearance, frequent multiple lesions, a relatively monotonous proliferation of centrocyte-like cells with reactive follicles, few other reactive components, infrequent lymphoepithelial lesions, and dense infiltration of the submucosa.

　The t(11;18)(q21;q21) translocation is the most frequent chromosome aberration in extranodal MALT lymphoma, resulting in API2-MALT1 chimeric products. API2-MALT1 chimeric transcripts were detected in most of the cases examined. Based on these observations,API2-MALT1 chimeric transcripts may be a good marker to predict the relevance of antibiotic treatment for gastric MALT lymphoma.
