胃悪性リンパ腫の病理学的診断法と問題点

Pathological Methods and the Problem of Diagnosis of Gastric Malignant Lymphoma

要旨　悪性リンパ腫の生検診断においては，組織が小さいことや挫滅などのアーチファクトなどにより組織構築の判断が困難なことがある．反応性リンパ濾胞の胚中心はCD10（＋），bcl-2（－），Ki-67（＋）であり，これらを用いた染色で胚中心を同定し，それ以外の部位でのリンパ球系細胞のマーカーによるT，B細胞の分布を参考にして組織構築を把握し，異型リンパ球の分布や細胞異型度をH・E標本で判定することが重要である．そして，リンパ球様細胞が増殖した像を認めた場合，それらの細胞が比較的大型である場合にはサイトケラチンによる未分化癌の鑑別が，細胞が比較的小型の場合はcyclin D1によるマントル細胞リンパ腫の鑑別が必要である．また，Ki-67は悪性度判定の指標としても有用である．PCR法によるIgH再構成は必ずしもリンパ腫すべてにおいて陽性に検出されるわけでなく胃炎でも検出される場合があり，またp53遺伝子異常はリンパ腫では低率であるので，それらは診断の補助的手法としては有用であるがその評価には注意が必要である．

　We often have difficulty in diagnosis of gastric biopsy because of the smallness and compression of the sample. In case of differential diagnosis of malignant lymphoma, it is important to detect the reactive germinal centers and clarify the histologic architecture. After detecting the reactive germinal centers which are immunohistochemically CD 10 (＋), bcl-2 (－) and Ki-67 (＋), the atypia of infiltrating lymphoid cells around the germinal centers should be evaluated on H・E section.

　When the infiltrating cells are relatively large, carcinoma should be ruled out by immunohistochemical stain using cytokeratins. When they are relatively small, mantle cell lymphoma should be ruled out by immunohistochemical stain using cyclin D1. In addition, Ki-67 is also useful for evaluating the grade of lymphoma.

　IgH rearrangement by the PCR method is not always positive for lymphoma and it can also be positive for chronic gastritis. p53 mutation has low incidence in lymphoma. These molecular methods may be helpful for diagnosis of lymphoma, however, they should be carefully evaluated.
