要旨　十二指腸に生じる代表的な非腫瘍性病変について，内視鏡所見による診断と鑑別診断を中心に概説した．異所性胃粘膜は最もよく遭遇する病変であり，多発病変では比較的診断は容易であるが，単発病変では他の腫瘍性疾患（腺腫，早期癌，カルチノイドなど）との鑑別が問題となることがある．また，粘膜下腫瘍様の形態を呈する異所性膵では，上皮性腫瘍であるカルチノイドとの鑑別が必要であり，ボーリング生検や超音波内視鏡が診断の一助となる．アミロイドーシスなどのびまん性疾患では，内視鏡像のみでは特殊なリンパ系腫瘍や感染症との鑑別は必ずしも容易ではなく，これらの疾患を念頭に置き病理（生検）診断を依頼することが重要である．

　During endoscopic examination, HGM（heterotopic gastric mucosa）is a non-neoplastic lesion which is most often encountered in the duodenum. Although endoscopic diagnosis of HGM composed of multiple lesions is comparatively easy, the one composed of a single lesion sometimes needs differential diagnosis with other neoplastic tumors, such as adenoma, early cancer and carcinoid tumor. Drilling biopsy and endoscopic ultrasonography are useful for the differential diagnosis of ectopic pancreas and carcinoid tumor. Furthermore, in differential diagnosis of diffuse mucosal lesions in the duodenum such as amyloidosis, infectious disease and a certain kind of malignant lymphoma, it is important to bear these diseases in mind and obtain from the pathologist a histological diagnosis of the biopsy specimen.
