要旨　現在，早期胃癌の治療においてESD（endoscopic submucosal dissection）やEMR（endoscopic mucosal resection）の手技が一般化してきている．それに伴い早期胃癌の内視鏡診断の重要性が増してきている．また生検は癌の診断のほかに，negative biopsyを施行して癌の範囲を確定させることで，ESD, EMRの適応か否かの鑑別の一助となっている．しかし，生検においては時折，標本の取り違いや，標本作製の不備などにより，時としてインシデントやアクシデントが起こりうるという現実がある．そのようなことを起こさないようにするためにも病理診断においては，日頃からの内視鏡医と病理医の連携が大切であり，内視鏡医は病理診断について，病理医は内視鏡診断についての知識が必要である．ここでは，一般的な生検材料の取り扱いから，病理診断までの流れ，生検に際し注意すべきこと，また早期胃癌の診断，治療の際に必要となる生検の基礎的な知識などについて症例を交えながら初歩的なことを中心に簡単に述べることとする．

　Recent advances in treatment for early gastric cancer have enabled endoscopic submucosal dissection（ESD）and endoscopic mucosal resection（EMR）. Endoscopic diagnosis is one of the most important issues of gastric cancer diagnosis. Biopsy under endoscopic procedures contributes not only to histological typing of gastric cancer, but also to determining its mucosal extension. Based on the pathological results of the biopsy, endoscopists and gastroenterologists perform ESD and EMR. Therefore, they have to understand adequate manipulations of gastric biopsy before performing ESD and EMR. In this paper, we describe how to manipulate specimens of the gastric biopsy, and explain things to be careful about when making the pathological diagnosis.
