要旨　症例：22歳，女性．既往歴：3歳時よりアトピー性皮膚炎．現病歴：大学受験時に心窩部痛があったが，受験後軽快．入学後も試験前に心窩部痛が出現し，試験終了後消失を繰り返していた．2000年4月試験終了後も消失しない心窩部痛があったため，近医受診．内視鏡検査を受け多発性胃潰瘍にて当科紹介受診した．身体所見・血液データ：アトピー性皮膚，心窩部に圧痛を認める以外特記すべきことなし．好酸球5.3％，IgE 1,400IU/ml以外特記すべきことなし．内視鏡所見：胃体部全周が陥凹し，白色調を呈し，多発性，散在性に小ポリープ様隆起性病変を認めた．H. pylori：培養，UBT，血清抗体，検鏡ともに陰性であった．PGI/II比は4.35であった．病理所見：内視鏡下生検にて被蓋上皮直下に好酸性物質が帯状に沈着し，Azan染色で強く青に染まり，アミロイド染色は陰性で，collagen bandであった．下部消化管内視鏡検査：全大腸内視鏡検査にて内視鏡的には正常，step biopsyによる病理組織にはcollagen bandは証明されなかった．以上より本症例はcollagenous gastritisと診断した．5年後の内視鏡所見および内視鏡生検病理組織所見は増悪・軽快なく，変化はなかった．

　Collagenous gastritis, with or without collagenous colitis, is a rare disorder with less than 20 cases reported in the literature. Collagenous gastritis is a distinctive disorder characterized by thickening of the subepithelial collagen band in the gastric mucosa and inflammatory cell infiltration of the lamina propria. A case of collagenous gastritis, without collagenous colitis, in a 22-year-old woman with epigastralgia is reported. Endoscopy revealed a depression area in the gastric body. Biopsy specimens showed a thickened eosinophilic band-like structure (approximately 30～70μm thick) beneath the surface epithelium. It was positive on Azan staining but negative on amyloid staining, and was accompanied by marked infiltration of mononuclear cells and eosinophilia in the lamina propria. Endoscopic and histological findings have not changed for 5 years.
