要旨　ITナイフ（insulation-tipped diathermic knife）は，病変の一括切除を目的として最初に開発および臨床応用された内視鏡的切開器具である．高周波針状ナイフの先端にセラミック製の小球を装着したナイフであり，絶縁チップにより，穿孔の危険性を減少させ，安全に切開および粘膜下層を剝離することができる．他のESD処置具との違いは，先端の絶縁体からシースまでの部分で切開することである．長所は粘膜下層を一気に剝離可能であり，他のデバイスに比べ術時間が短い点であり，短所は，ブラインドになる場合があること，粘膜下層を直視してまっすぐ前方を切開することができないことである．

　We developed a special endoscopic knife in 1995 named “insulation-tipped diathermic knife (IT knife)”, in which we tipped a ceramic ball with the top of a needle knife. This knife can cut submucosa safely and remove a lesion completely. Now in Japan, besides the IT knife, several other types of knives have been developed and are used. Dissection with these knives is called “enodscopic submucosal dissection”. Although the IT knife has some weak points, namely technical difficulty in its use and bleeding due to incised submucosa, all patients affected received conservative treatment and none underwent emergent surgery. One-piece resection makes possible a correct recommendation for patients with early gastric cancer and the IT knife and other special knives make this possible.
