Endoscopic Submucosal Dissection Using a Flex Knife for the Treatment of Early Gastric Cancer

要旨　フレックスナイフの最大の特徴はその柔軟性にある．ナイフ先端の長さの調節を慎重に行い，深く潜り込まないようにストッパーを利用することで，穿孔の危険の少ない安全な切開・剝離を行うことが可能である．また，この処置具1本でESDを完遂することができ，コストパフォーマンスにも優れている．ナイフの特徴やウォータージェット機能，粘膜下局注法，止血術や後処置などを十分に熟知した上でESDを導入することにより，安全で確実な治療が広く普及していくことが望まれる．

　The most important feature of the Flex knife is its flexibility. It is possible to incise and dissect safely with minimum risk of perforation by carefully adjusting the length of the knife and using the stopper so as not to slip too deeply. Moreover, the Flex knife is cost-effective, since ESD can be completed with this single device. ESD should be introduced after acquiring thorough knowledge and skill in manipulating it for treatment, including the submucosal injection method, hemostasis and treatment of the ulcer bed after resection.

　It is desirble that this new exceptionally effective technique becomes used generally.
