中・下咽頭癌の病理―食道癌の病理と比較して

Pathology of Oro-and Hypopharyngeal Cancer

要旨　中・下咽頭における扁平上皮癌はこれまで進行した状態で見つかることが多く，頭頸部癌の中では予後の悪いものの1つであったが，内視鏡的診断技術の進歩により，早期扁平上皮癌が見つけ出されるようになってきた．病理形態学的には食道扁平上皮癌と基本的に同じような診断基準で診断可能と考えられるが，解剖学的に粘膜筋板が存在しないために，早期中・下咽頭癌または表在扁平上皮癌の定義が困難である．したがって，粘膜切除された表在性扁平上皮癌はこれから経過を観察する必要がある．現在，表在性中・下咽頭癌症例の蓄積が急速に進んでおり，癌の深達度や組織形態像とリンパ節転移との臨床病理学的解析を行い，中・下咽頭癌における標準的な内視鏡的治療の臨床病理の確立が必要とされている．

　Squamouse cell carcinoma of the oropharyngeal and hypopharyngeal mucosal sites has been detected clinically at advanced stages and generally has a poor prognosis. Recent endoscopical development increasingly enables us to detect superficial squamous cell carcinoma at the oropharyngeal and hypopharyngeal sites. Although histological diagnosis of superficial oro- and hypopharyngeal cancer can be made in the same way as that for esophageal cancer, the definition of depth of invasion has not been defined yet because, anatomically, there is no muscularis mucosae. Clinical follow up is mandatory for cases which are recently being treated endoscopically. Futher clinicopathological analyses will be needed for the establishment of standard treatment of superficial oropharyngeal and hypopharyngeal squamous cell carcinomas.
