会陰部に発症したHPV16型陽性のBowen癌

・会陰部に中心が白色調で辺縁が黒色〜灰白色調の特異な臨床像を呈した腫瘍がみられ，病理組織学的所見よりBowen癌と診断した．

・ウイルス感染を示唆する病理組織学的所見があり，ウイルス学的検索を行ったところ，ヒトパピローマウイルス（human papilloma virus：HPV）16型が検出された．

・p16蛋白に対する免疫染色では，腫瘍細胞がびまん性に陽性であった．HPV16型遺伝子の発現が，腫瘍細胞の増殖に強く関わっていると考えた．

A case of perineal Bowen’s carcinoma with human papillomavirus type 16 infection

We report a 75-year-old woman who presented with a 4-month history of perineal nodules and pain on urination. Physical examination revealed a blackish-brown nodule on the lower side of the vulva. Histopathological examination of the nodule demonstrated atypical keratinocytes distributed irregularly from the epidermis to the dermis and koilocyte-like change in the superficial part of the epidermis. The lesion showed positive immunostaining for p16. Human papillomavirus (HPV)type 16 was detected from a specimen of the nodule using polymerase chain reaction, suggesting that HPV had played a major role in carcinogenesis.
