腋窩，鎖骨上リンパ節，肺転移をきたした右側胸部原発基底細胞癌

・原発巣切除6年後に所属リンパ節転移，肺転移をきたした基底細胞癌を経験した．転移は非常にまれであるが，おこしうることを念頭に置いて診療に臨むべきである．

・リンパ節転移に対して所属リンパ節郭清，術後補助療法として放射線照射を行った．

・単発の肺転移に対して局所切除を行い経過観察中である．

A case of primary basal cell carcinoma of right chest with axillar, supraclavicular lymph nodes, and lung metastasis

Six years before the first visit, a man in his 50s had developed a 13 mm of black tumor on his right side chest. Histopathological diagnosis of the excised tumor was basal cell carcinoma (BCC). At 5 years and 6 months after surgery, right axillar and supraclavicular lymphadenopathy appeared, and the patient was referred to our hospital for the lymph node biopsy of suspected of BCC metastasis. No obvious organ metastasis was detected by FDG-PET and the abnormal accumulation was limited to the enlarged lymph nodes. The radical lymphadenectomy was performed followed by postoperative radiation. One year later, whole body CT scan showed a nodular shadow increasing in the lower lobe of the right lung. Excision of that part was performed and it was proved to be BCC metastasis.
