A 75-year-old man developed whitish subcutaneous nodule on the dorsal side of the second distal interphalangeal joint. He has never experienced any kind of pain in the affected area. Histological examination of the nodule revealed gouty tophus. During the biopsy, the tophus was removed through débridement. I think that development of tophus may have been brought about by stagnation of the bloodstream around the second distal interphalangeal joint. The Grecian foot deformity of the pharanx bones or additional trauma may have possibly caused stagnation of bloodstream.
