Abstract A 2-year-old girl with a verrucous nodule on the left thumb and multiple small papules on the face and perianal region was referred to our hospital. Since human papillomavirus types 2 (HPV-2) was detected from the eruption on the finger and perianal region, she was diagnosed with verruca vulgaris by HPV-2. She was refractory to usual treatments for verruca vulgaris; however, topical imiquimod treatment was remarkably effective. When we encounter verrucous lesions on the perianal region, we should include verruca vulgaris as differential diagnosis. Although the treatment of topical imiquimod is not available for verruca vulgaris in Japan, the treatment might be considered in some refractory cases.
