 While most gastric cancers arise in the setting of chronic gastritis due to H. pylori infection, the diagnosis of gastric tumors in H. pylori-uninfected patients has been increasing in recent years due to declining H. pylori infection rates and the growing awareness of endoscopists. Gastric tumors in H. pylori-uninfected patients are often low-grade and confined to the mucosa. Additionally, these tumors, including foveolar-type neoplasms, exhibit distinct histological features compared to those arising in the background of chronic gastritis. This review provides an overview of the histological and molecular features of gastric tumors in H. pylori uninfected patients.
