要旨

近年の内視鏡の飛躍的な進歩に伴い，低侵襲な治療として消化器疾患診療の中心的な役割を果たすようになった消化器内視鏡ではあるが，偶発症としての出血は解決すべき問題の一つである。抗血栓薬内服による出血のリスクと休薬による血栓塞栓症のリスク，両者のバランスを取りながら消化器内視鏡診療を行う必要がある。しかし，ガイドラインを遵守しても起こりうるトラブルに対してどのように対応するべきか，依然として課題の残る領域ではあるが，その歴史的な変遷や現状，そしてトラブルシューティングについて本稿で解説する。

　With the rapid progress of endoscopes in recent years, gastrointestinal endoscopy has come to play a central role in the treatment of gastrointestinal diseases as a minimally invasive treatment, but accidental bleeding is still a problem to be solved. Gastrointestinal endoscopic practice must balance the risk of bleeding due to oral antithrombotic agents and the risk of thromboembolism due to drug withdrawal.However, how to deal with possible problems that may occur even in compliance with guidelines remains an area of challenge, and this article describes the historical evolution, current status, and troubleshooting.
