　副甲状腺機能亢進症は，腺腫や副甲状腺癌による原発性機能亢進症と透析後に発症する過形成による二次性機能亢進症がある。それらの治療は主に外科切除であり，患者への侵襲を減らすために正確な病変の存在診断・部位診断が重要である。本稿では，副甲状腺機能亢進症における画像診断である超音波検査と99mTc-2methoxy-2methylpropylisonitrile（99mTc-MIBI）シンチグラフィ検査について長所，短所，及び診断能について説明する。近年普及しつつあるSPECT/CTの有用性についても言及した。

　Treatments for primary hyperparathyroidism due to adenoma, hyperplasia and carcinoma and secondary hyperparathyroidism are mainly surgical resections of them. Accurate imaging diagnoses of the existences and the regions are very important for reductions of invasiveness. We describe ultrasonography and 99mTc-MIBI scintigraphy of hyperparathyroidism. We explain an advantage, a disadvantage and diagnosability of these modalities. We mention utilities of SPECT/CT, too. We show echogram and 99mTc-MIBI scintigraphy images about 3 cases of hyperparathyroidism.
