３週間以上続く咳嗽で初診する喘息・咳喘息に対する低用量のVFC（vilanterol fluticasone combination：レルベア®）およびFFC（formoterol fluticasone combination：フルティフォーム®）の有用性

Advantage of low dose vilanterol fluticasone combination and formoterol fluticasone combination in the treatment of persistent cough as a initial complaint of bronchial asthma or cough variant asthma

３週間以上続く咳を主訴に初診し，最終的に咳優位型喘息もしくは咳喘息と診断された92例に対して，最初の２週間に投与した各種controllerの効果について検討した。夜中の咳に対しては，FBC（formoterol budesonide combination dry powder） ４吸入，SFC（salmeterol fluticasone combination dry powder） 200，VFC（vilanterol fluticasone combination dry powder） 100，FFC（formoterol fluticasone combination aerozol） 200-spacer使用の4種が，他のcontrollerによる治療よりも，有意に短い日数で咳が消失した。１日全体の咳に対しては，VFC 100とFFC 200-spacerが，最も効果が早く，SFC 200よりもVFC 100のほうが，FFCでは直接吸入よりもspacer使用のほうが，有意に効果が早かった。咳で初診する喘息にも将来のリモデリングの予防とアドヒアランスの向上のために，効果の早いICS/LABAの使用が望ましい。VFC 100が最も効果が早く，FFC 200はスペーサー使用で同等の効果があった。

　We investigated the effect of low dose VFC（vilanterol fluticasone combination）dry powder and low dose FFC（formoterol fluticasone combination）aerosol metered-dose inhaler on persistent cough as initial chief complaints of 92 patients with bronchial asthma or cough variant asthma compared with other controllers（pranlukast, pranlukast with tulobuterol patch, mometasone furoate and formoterol budesonide combination）. Low dose VFC dry powder and low dose FFC aerosol with tube-type spacer improved most quickly both cough at night and all day cough among these controllers. We concluded that strong inhaled ICS/LABAs are suitable for early intervention of bronchial asthma and preventing airway remodeling.
