はじめに
周術期等口腔機能管理(図1)は,医科疾患治療時,主に全身麻酔下での手術時の合併症予防を目的として,主に病院歯科で行われている1)2)。口腔衛生管理によって誤嚥性肺炎の発症率が減少することが報告3)されて以降,わが国では2000年ころよりその重要性が医療界で広く認知されるようになった。周術期において口腔衛生状態を管理することによって,術後の創部感染や肺炎などの合併症の発生リスクと術後在院日数や医療費が減少することが明らかにされてきている1)3)~5)(図2,3)。例えば,がん治療(手術)では,口腔の衛生管理による入院期間の短縮,術後肺炎の減少,創部感染の減少が,口腔がん6)7),食道がん8)~11),肺がん4)12)~14),肝臓がん15)について報告されている。本論文では,主に術後肺炎予防の周術期口腔衛生管理ならびに,歯科,麻酔科,看護の連携について述べる。
Perioperative oral management is performed primarily at dental facilities with the goal of preventing complications during patients’ medical treatment. The importance of providing perioperative oral management via oral care has been widely recognized in Japan’s medical community since~2000, as this management has been reported to reduce the incidence of aspiration pneumonia. Perioperative oral management has been demonstrated to reduce the risk of postoperative wound infection and aspiration pneumonia and to reduce the length of patients’ postoperative hospital stays. For example, perioperative oral management has been reported to shorten hospital stays and reduce the rates of postoperative pneumonia and wound infection in cases of oral, esophageal, lung, and liver cancers. This article describes(i)techniques for implementing perioperative oral care and(ii)a collaborative protocol for reducing the risk of postoperative complications based on collaborations among dentists, anesthesiologists, and nurses.
