はじめに

　フレイル患者と術後合併症というキーワードでの文献検索において，過去10年間で関連する文献数が急激に増加している。フレイルが周術期の予後不良因子となる手術や病態としては，各種の悪性腫瘍手術（食道がん，肝臓がん，膵臓がん，胃がん，肺がんなど），心臓手術，生体肝移植手術などが挙げられる。また，各臓器別の合併症としても，術後肺炎の増加や手術部位感染（surgical site infection：SSI）を始めとして，中枢神経系，呼吸器系，循環器系など，複数の術後合併症との関係性が推測される。本論文では，これらの合併症に関する知識を整理し，術中管理・術後管理での注意点や対応について考える。

　Searching the scientific literature by using the keywords “frailty” and “postoperative complications” reveals that the number of reports covering these topics has rapidly increased over the past 10 years. Examples of surgeries and conditions in which frailty is a poor perioperative prognostic factor include surgeries for multiple types of cancer（e. g., esophageal, liver, pancreatic, gastric, and lung cancers）, cardiac surgery, and living-donor liver transplant surgery. In terms of organ-specific complications, frailty is also thought to be related to multiple postoperative complications, including increased risks of postoperative pneumonia and surgical site infections（SSIs）, as well as complications in the central nervous system, respiratory system, and circulatory system. The present article summarizes the current knowledge about these complications and considers precautions and responses during and after surgery for frail patients.
