 Female patient with asymptomatic hypercreatinekinasemia underwent paranasal sinus surgery under general anesthesia. Immediately after the discontinuation of the inhalational anesthetic agent, she developed tachypnea, tachycardia, and impaired consciousness, accompanied by a rapid increase in body temperature to 39.1℃. An arterial blood gas analysis revealed acidosis. Postoperative malignant hyperthermia was suspected and immediate treatment was initiated, including an intravenous administration of dantrolene, re-intubation, high-flow oxygen, and external cooling. Her condition improved rapidly, and she was successfully extubated the following day. Next-generation panel sequencing for malignant hyperthermia(MH)did not reveal any pathogenic variations in the known causative MH genes.
