 Acute kidney injury(AKI)is a frequent complication of major surgery, and the question of how best to prevent perioperative AKI and avoid severe complications remains an important issue. The Kidney Disease Improving Global Outcomes(KDIGO)bundle is the intervention bundle described in the KDIGO practice guidelines for AKIs. It lists six interventions for patients at high risk of developing an AKI:(1)discontinue nephrotoxic substances whenever possible,(2)ensure the appropriate fluid volume and perfusion pressure,(3)consider functional hemodynamic monitoring,(4)monitor the patient’s serum creatinine levels and urine output,(5)prevent hyperglycemia, and(6)consider non-contrast alternatives for imaging. KDIGO practice guideline also suggest that renal replacement therapy should be considered for patients at AKI stage ≧2. The use of the KDIGO bundle has been shown to reduce the frequency of AKI and major adverse kidney events in patients undergoing cardiac surgery and extensive surgery, and this use is now considered a standard prevention and treatment strategy.

 In addition, biomarkers reflecting renal tubular stress and the application of remote ischemic preconditioning as a means of preventing AKI are attracting attention and are summarized in this series.

 The current thematic provides an overview of the latest findings on the prevention and treatment of perioperative acute kidney injury.
