要旨

　先行留置した中心静脈カテーテル（central venous catheter：CVC）と同一部位から一時的ペーシングリードの追加穿刺を行い，CVCを穿通した症例を経験した。同一部位から穿刺する場合は先行留置したCVCを損傷しないために，超音波とX線透視の併用下に，穿刺血管，CVC，穿刺針，ガイドワイヤーなどすべてを視認しながら施行する必要がある。

　We report the accidental puncture of a previously placed central venous catheter（CVC）during the insertion of a temporary pacing lead.

　The patient underwent transcatheter aortic valve replacement under general anesthesia and a CVC was inserted under ultrasonic（US）guidance. During the surgical procedure, the patient required temporary pacing and the right internal jugular vein was punctured under US guidance. The guidewire penetrated the CVC.

　Although guidewires are visible on US, CVCs are invisible. When performing a puncture near a CVC, we recommend using fluoroscopy in addition to US guidance.
