Ultrasound guided central venous canulation and port implantation for cancer patients and palliative care patients focusing on micro-puncture technique

中心静脈カテーテルや中心静脈ポート留置術は，抗がん剤治療および中心静脈栄養を安全に行うための重要な医療行為である。がん患者および緩和ケアを要する患者はいずれも，るい痩，血管内脱水，血液凝固障害などの全身状態が良くないことがめずらしくなく，気胸や動脈誤穿刺などの合併症を引き起こさないよう，慎重な適応決定と手技施行が求められる。合併症回避に画像ガイド下穿刺術は有効な手技であり，医療安全の観点からも推奨されている。本稿では，中心静脈穿刺における超音波リアルガイド下穿刺とマイクロパンクチャーテクニックの工夫を主体として，中心静脈カテーテルおよびポート留置術を筆者の経験を含めて概説する。

Central venipuncture, central venous catheterization, and central venous port placement are important medical procedures for cancer patients and palliative care patients to administer anticancer drugs and to manage transvenous nutrition. These procedures can have various complications, such as pneumothorax and arterial malperforation, and should be approached with an awareness of the high risk associated with these procedures. The use of micropuncture techniques, along with an understanding of puncture location and ultrasound real-guided puncture, will help ensure safe procedures and reduce complications.
