要旨

　重度の肺気腫を合併した続発性気胸の手術において，静脈脱血静脈送血体外式膜型人工肺（veno-venous extracorporeal membrane oxygenation：V-V ECMO）を併用し，麻酔管理を行った。抗凝固薬の全身投与は行わなかった。手術時間は8時間6分，出血量は1,734gに及んだがECMO流量は維持され，酸素化も良好であった。血栓性合併症は生じなかった。手術終了後，速やかにECMOを離脱した。

　A 70-year-old man underwent surgery for secondary pneumothorax complicated with severe emphysema, anesthesia management was performed in conjunction with veno-venous extracorporeal membrane oxygenation（V-V ECMO）. No systemic anticoagulants were administered. The operative time was 8 hours and 6 minutes, and the total blood loss was 1734 g. ECMO flow was maintained and oxygenation was favorable. There were no thrombotic complications and the patient was weaned off ECMO within 1 hour after surgery.
