A 70-year-old man underwent surgery for secondary pneumothorax complicated with severe emphysema, anesthesia management was performed in conjunction with veno-venous extracorporeal membrane oxygenation(V-V ECMO). No systemic anticoagulants were administered. The operative time was 8 hours and 6 minutes, and the total blood loss was 1734 g. ECMO flow was maintained and oxygenation was favorable. There were no thrombotic complications and the patient was weaned off ECMO within 1 hour after surgery.
