はじめに

　急性呼吸不全や心原性ショックに対する，体外式膜型人工肺（extracorporeal membrane oxygenation：ECMO）による呼吸・循環補助は，重症病態時の使用が近年増加してきた。しかし，麻酔科医が対応する困難気道管理（difficult airway management：DAM）において，ECMOをどのような立ち位置としてとらえるか，現状では明確なコンセンサスが確立していない。DAMにおけるECMOの適応を検討するための背景知識として，本論文では各国のDAMガイドラインにおけるECMOの位置づけについて概説を行う（表）。

　There is currently no consensus regarding precisely how and in what circumstances extracorporeal membrane oxygenation（ECMO）should be applied in patient cases that involve difficult airway management（DAM）. This article provides an overview of different countries’ guidelines concerning the use ECMO in DAM cases. It is hoped that clear and detailed guidelines for the implementation of ECMO in DAM strategies will be developed in the future.
