 Pulmonary alveolar proteinosis is a rare syndrome characterized by the abnormal accumulation of surfactant-derived material within the alveoli. The standard treatment is whole lung lavage, and extracorporeal membrane oxygenation is an effective support for patients with severe hypoxemia. We describe the case of a patient with pulmonary alveolar proteinosis and low cardiac function for whom extracorporeal membrane oxygenation(ECMO)was introduced while the patient was awake, allowing for the safe execution of whole lung lavage.
