 Cardio-facio-cutaneous syndrome is a rare condition characterized by craniofacial features, congenital heart disease, seizure, musculoskeletal manifestation, growth retardation and intellectual disability. These conditions may significantly impact patient perioperative outcomes.

 We describe a case of 13 year-old child with cardio-facio-cutaneous syndrome who has a need of feeding tube for a long term. Gastrostomy was scheduled under general anesthesia using muscle relaxant with neuromuscular monitoring. The onset of muscle relaxant was delayed and the duration of its effect was not prolonged. Furthermore, rapid recovery of muscular function was gained with sugammadex.

 No postoperative respiratory complications were observed during perioperative period in this case.
