要旨

　急速に増大する巨大卵巣腫瘍により腹腔内圧（intraabdominal pressure：IAP）が上昇し腹部コンパートメント症候群（abdominal compartment syndrome：ACS）を発症した高度肥満の1症例を経験した。本症例は，IAP減圧前に筋弛緩薬を投与したことで呼吸・循環管理に難渋した。ACS患者においては，筋弛緩薬を投与することでIAPは低下するが，胸腔内圧が大きく上昇する。安定した呼吸・循環管理を維持しながら，IAP低下を目標に安全かつ効率的なACSの全身管理を達成するためには，筋弛緩薬の投与タイミングが重要であると考える。

　We experienced a case of obese patient with ACS（abdominal compartment syndrome）due to IAP（intra abdominal pressure）caused by a rapidly growing large ovarian tumor. In this case, we administered muscle relaxants before decrease of IAP, that made it difficult to manage respiration and circulation. In patients with ACS, administration of muscle relaxants decreases IAP but greatly increases intrathoracic pressure. We consider that the timing of muscle relaxant administration is important for achieving safe and efficient systemic management of ACS, with the goal of lowering IAP while maintaining stable respiratory and circulatory control.
