 A 16-year-old female adolescent underwent sclerotherapy under general anesthesia for a shoulder venous malformation. Tracheal intubation and supraglottic airway insertion were avoided due to airway venous malformations. Anesthesia was maintained using a continuous intravenous infusion of propofol and intermittent fentanyl under spontaneous breathing. During anesthesia, 100% oxygen(flow-rate 60 l・min−1)was delivered via a high-flow nasal cannula(HFNC). The patient remained hemodynamically stable, and the oxygen saturation was maintained above 98% throughout the procedure. In this case, HFNC administration concomitant with total intravenous anesthesia was useful for maintaining airway patency.
