要旨
2019年に報告されたthoracoabdominal nerves through perichondrial approach block(TAPAB)は,T5からT12までの腹部前面から側面にかけての鎮痛が得られたと報告されている。われわれは硬膜外麻酔穿刺困難な開腹手術の周術期鎮痛に持続TAPABが有効であった症例を経験した。持続TAPABは腹部手術の鎮痛に有用である可能性が示唆された。
The thoracoabdominal nerves through perichondrial approach block(TAPAB), reported in 2019, provides analgesia from the anterior to lateral aspects of the abdomen from T5 to T12. We present a case wherein continuous TAPAB was effective for perioperative analgesia in an open abdominal surgery, for which epidural anesthesia was difficult. Compared with conventional peripheral nerve blocks, TAPAB requires fewer punctures and is safer as it is performed in shallow layers. Continuous TAPAB may be useful for postoperative analgesia in abdominal surgery.
