腹腔鏡下幽門側胃切除術における末梢神経ブロック併用術後鎮痛法の検討

Analgesic efficacy of peripheral nerve block after laparoscopic distal gastrectomy

◆要旨：胃癌患者に対する腹腔鏡下幽門側胃切除術の術後鎮痛法として，末梢神経ブロックと硬膜外麻酔の鎮痛効果をretrospectiveに検討した．対象は2009年3月～2010年12月に当科で施行した完全鏡視下幽門側胃切除術症例である．末梢神経ブロック群18例および硬膜外麻酔群32例の計50例について，術当日から術後5日目までのface rating scaleと追加鎮痛薬の使用回数を比較し，両群間に有意差を認めなかった．末梢神経ブロック群の12例に周術期抗凝固療法が施行されていた．術後に静脈血栓塞栓症や出血性合併症を生じた症例はなかった．硬膜外麻酔に代わる安全性の高い術後鎮痛法として，末梢神経ブロックの有用性が示唆された．

We evaluated the efficacy of peripheral nerve block compared with epidural anesthesia after laparoscopic distal gastrectomy （LDG） for gastric cancer. Between March 2009 and December 2010, 18 patients underwent LDG with peripheral nerve block, and 32 patients with epidural anesthesia. Face rating scale and the number of additional analgesic administration per patient in the peripheral nerve block group were not significantly different from that in the epidural anesthesia group. There were no patients with venous thromboembolism or bleeding complications in both groups. In conclusion, peripheral nerve block seems to be an effective and safty analgesia of LDG.
