悪性腫瘍が原因と判明した上肢痛の2症例

Two Cases of Upper Extremity Pain Caused by Malignant Tumor

要旨

　上肢痛の原因が肺尖部肺がんによるパンコースト症候群，乳がんのリンパ節転移による腕神経叢障害であった2症例を経験した。慢性痛における上肢痛の鑑別診断を行ううえで悪性腫瘍が関連している可能性に留意する必要があり，その特徴を理解しておく必要がある。

　We report two cases of upper extremity pain related to the malignant tumor. Case 1：A man presented severe pain with allodynia, edema, and atrophy at his right upper extremity. He has been diagnosed with Pancoast syndrome caused by an apical lung tumor. Case 2：A woman presented pain, edema, and muscle weakness at her right upper extremity. She had undergone bilateral mastectomy for breast cancer, with diagnosed complete remission. But we revealed that the cause of the pain was brachial plexus compression due to lymph node recurrence. It is essential to exclude “red flags” for diagnosis in clinical settings.
