 An 88-year-old woman presented with a gradually enlarging vulvar mass that had been present for >50 years. The mass had eventually ulcerated and began to bleed, prompting her to visit her previous physician. She was diagnosed as having advanced vulvar cancer. Approximately 2 months later, she suffered massive bleeding from the tumor and was hospitalized for bed rest management. Due to her advanced age, curative surgery was not performed. Instead, tumor resection and vulvar reconstruction using a pedicled rectus abdominis musculocutaneous flap were performed with the aim of hemorrhage control. The patientʼs activities of daily living recovered to full independence within 3 days after the surgery. As tumor bleeding significantly impairs a patientʼs quality of life, resection and reconstruction should be considered even in patients who are under palliative care.
