 Arteriovenous malformations (AVMs) often recur even when they are thought to have been completely removed, and a complete cure is thus rare. Accordingly, the goal of AVM treatment is generally to control rather than resolve the AVM. We describe the clinical details of an AVM in a 24 -year-old man patientʼs thumb that had been surgically removed during childhood that recurred 20 years later and required treatment. Reconstruction with a free flap was performed, but delayed necrosis of the flap necessitated reoperation. The patient has since been able to resume a normal life with no sequalae. This case highlights the difficulty of the long-term control of an AVM in a thumb.
