はじめに

　AVMは完全に切除したと思っても再発することが多く，完治するのはまれである。AVMは解剖学的にいくつかの組織を含んでびまん性に存在しているので，完全切除は困難である。AVMの治療のゴールは，一般的には完治よりもコントロールすることである 1）。そのため，治療を行う場合は再発を考慮して，長期的な経過を予想しながら計画しなければならない。

　今回，小児期に切除した母指のAVMが20年後に再発した症例を経験した。遊離皮弁による再建を施行したが，遅発的に皮弁壊死を生じて治療に難渋したため，報告する。

　Arteriovenous malformations (AVMs) often recur even when they are thought to have been completely removed, and a complete cure is thus rare. Accordingly, the goal of AVM treatment is generally to control rather than resolve the AVM. We describe the clinical details of an AVM in a 24 -year-old man patientʼs thumb that had been surgically removed during childhood that recurred 20 years later and required treatment. Reconstruction with a free flap was performed, but delayed necrosis of the flap necessitated reoperation. The patient has since been able to resume a normal life with no sequalae. This case highlights the difficulty of the long-term control of an AVM in a thumb.
