 A surgical microscope is the most precise system available in clinical practice for detailed observation, and its utility is particularly high in eyelid surgery. Based on the authorʼs experience, the use of a surgical microscope provides significantly clearer visualization of the operative field compared to the naked eye or surgical loupes, resulting in consistently better surgical outcomes. Lower eyelid entropion can be classified into epiblepharon and involutional entropion, each requiring distinct clinical observations and treatment approaches. This article focuses on the frequently encountered conditions of lower eyelid epiblepharon and involutional entropion, detailing their anatomical background and the use of a surgical microscope in their treatment. The clinical significance of reconstructing the subcutaneous branches of the capsulopalpebral fascia (CPF) in epiblepharon is discussed, along with the importance of a preoperative evaluation of epicanthal folds. For involutional entropion, the influence of horizontal eyelid laxity and CPF relaxation on the pathophysiology is explained, providing guidance on treatment selection.
