 Epiblepharon involves a normal eyelid position; only the eyelashes are turned towards the eyeball, unlike lower-eyelid entropion. The pathology involves an imbalance between the anterior and posterior lamellae. The skin perforators from the anterior layer of the lower eyelid retractor (LER) are underdeveloped, and the resulting excess skin and orbicularis oculi muscle ride backwards. However, cases of epiblepharon caused by caudal hypertraction of the posterior layer of the LER also exist. Although incision methods (mainly a modified Hotz procedure) are frequently used in the surgical procedure for epiblepharon, the incision method alone may result in recurrence in some cases. Toward the goal of reducing the rate of the recurrence of epiblepharon, various techniques are combined with a modified Hotz procedure. For example, lid-margin splitting with a modified Hotz procedure can easily evert the medial eyelashes. We believe that disinsertion of the LER can allow the tarsal plate to reposition cranially, effectively addressing the imbalance. By performing both lid-margin splitting and LER disinsertion with a modified Hotz procedure, it becomes possible to reduce the rate of epiblepharon recurrence.
