顕微鏡下の眼瞼下垂手術（2）—モノポーラ鑷子の使用—

Ptosis Surgery with Monopolar Forceps Viewed Through an Operating Microscope

はじめに

　著者が顕微鏡下にモノポーラ鑷子で眼瞼手術を始めたのは，2022年春にMIE眼科四日市の木下慎介先生と，蘇春堂形成外科の野平久仁彦先生の手術を見学させていただいたのがきっかけである。それまではルーペ下にて剪刀を用いた古典的な手術を行っていた。その経緯については本誌「私の心に残る1症例」No.50に記載した 1）。顕微鏡下に眼瞼手術を行うことにより，微細な出血に対する止血と縫合操作が可能となった。また，術者と助手や見学者へsurgeonʼs viewでの手術イメージの共有が可能になったことにより術者の身体的負担は軽減され，結果的に患者の満足度も上がり症例数の増加につながった。

　ルーペ下から顕微鏡下に移行する際には，機材や器具などを若干変更すると快適に手術を施行できる。本稿では，著者が行っている顕微鏡下のモノポーラ鑷子を用いた眼瞼下垂手術の挙筋腱膜前転法の実際について報告する。

　This article delineates methodology that is applied in levator aponeurosis advancement, i.e., using a monopolar forceps under an operating microscope. Microsurgery of the eyelid allows the surgeon to control haemostasis and manipulate sutures in the event of minor haemorrhage. It also permits surgeons to share their view of the operation with assistants and observers, thereby reducing the physical burden on the surgeon and resulting in increased patient satisfaction and an increased number of cases. This method has resulted in a notable increase in the number of cases at our department. The SK forceps, a monopolar settlement-type device designed primarily for use in eyelid surgery, has proven to be a valuable tool in this field due to its ability to function as a settlement when not energised and its ability to make an incision by closing, provide coagulation by grasping, and perform tearing and burning with a single device.
