 Chronic limb-threatening ischemia (CLTI) is defined as severe chronic ischemia resulting in chronic rest pain, ulceration, and necrosis. CLTI has a very high mortality rate. We describe a case in which we observed improvement in blood flow and ulceration after treatment with Collategene Ⓡ (beperminogene perplasmid). The patient was a 63-year-old male who had an ulcer on the right first toe and pain at rest. He was referred to our hospital after endovascular therapy was performed, with no improvement. At his initial visit, a 15 mm x 15 mm ulcer was observed on the right first toe, and treatment with Collategene Ⓡ was initiated. After two doses of Collategene Ⓡ had been administered, the blood flow and ulceration improved. The ulcer healed by 9 months after this treatment, and there has been no recurrence of the ulcer since then.
