腹部手術後に長期経過して自覚症状を認めたSchloffer腫瘤の3例

Three Cases of Schlofferʼs Tumor with Subjective Symptoms That Occurred Long After Abdominal Surgery

はじめに

　Schloffer腫瘤は,腹部手術後に残存した縫合糸やガーゼ片などの異物周囲に，長期間経過してから肉芽組織増生と膿瘍形成される異物性慢性炎症性肉芽腫である 1）。腹部手術後の瘢痕近傍に長期間経過してから腫瘤形成や膿瘍形成が認められた際にはSchloffer腫瘤も念頭に置く必要がある。

　今回われわれは，Schloffer腫瘤の3例を経験したので報告する。

　Schlofferʼs tumor is also called ʻforeign body chronic inflammatory granuloma.ʼ Inflammatory granulation tissue growth is observed around foreign objects such as sutures or pieces of gauze left in open surgical wounds, and abscesses are thus thought to occur over a long period of time. Since a mass is found near the wound in such cases, the metastasis of malignant tumors and desmoid tumors are differentiated. However, if (i) the patient has a history of surgery around the mass, (ii) subjective symptoms due to the same site are observed, and (iii) the mass is prolonged, it is important to also consider Schlofferʼs tumor as a differentiator. We treated three patients (two male and one female, mean age of 61.3±6.1 years old) with a Schlofferʼs tumor with subjective symptoms that occurred mean 26.7 years after abdominal surgery.
