 Pyoderma gangrenosum(PG)causes chronic and progressive skin necrosis and ulceration of unknown etiology. The estimated annual incidence of PG in Japan is 3 to 10 per million people, making it a relatively rare disease. There are few established methods for diagnosing and treating PG, and it tends to be difficult to treat. Surgical treatment alone is generally contraindicated, and local topical therapy or a systemic administration of steroids or immunosuppressive agents are indicated depending on the severity of the disease. Adalimumab(ADA), an anti-tumor necrosis factor-alpha(TNFα)monoclonal antibody, was approved in Japan for the treatment of PG in December 2020, expanding the available options. This report describes the first case of bilateral lower leg PG treated with autologous skin grafting and ADA, with good results.
