急性期と慢性期のハイブリッド医療を目指して

Hybrid Treatment of Acute and Chronic Wounds

はじめに

　近年，医療経済的観点から在院日数の短縮が推奨され，一方では再生医療や新たなデバイスの出現などにより創傷治療は飛躍的に進歩している。しかし，それでも慢性創傷の治療には時間を要し，その対応に苦慮することが多い。このため，創処置のそのものは短時間ですむ患者でも，通院困難であるという理由で入院が長期化する場合がある。そのような場合，「在宅医療」という選択肢はどうだろうか。

　著者は急性期と慢性期のハイブリッド医療を目指して，急性期病院の常勤医として勤務しつつ創傷専門の訪問診療クリニックを開設した。急性期病院では気付かなかった在宅医療ならではの治療というものがあることを経験したので，症例を供覧しつつその有用性について述べる。

　In-home care was introduced to provide the hybrid treatment of acute and chronic wounds, i.e., at both a healthcare facility and in the patient's home. The advantages of the in-home care component of this hybrid treatment include less time restrictions compared to hospitalization; continued treatment in close collaboration with the hospital; the ability to provide emergency care at a hospital if the patient's condition deteriorates; the benefit of having different physicians at home and at the hospital; and the ability to allow patients who have restricted visitation due to the COVID-19 pandemic to live at home with their family. The hybrid treatment of acute and chronic wounds is expected to be a positive new approach to wound management.
