鼠径部の難治性リンパ漏に対して顕微鏡下にリンパ漏開口部のZ縫合とリンパ囊胞被膜弁の被覆にて治癒した1例

Microsurgical Ligation and a Capsule Flap of Lymphocele for Intractable Inguinal Lymphatic Leakage

はじめに

　リンパ節郭清術後に生じる頻度の高い合併症として，リンパ漏がある。リンパ漏を生じると，リンパ液の皮膚からの流出，皮下へのリンパ液貯留（リンパ囊胞の形成），創の縫合不全，創縁壊死，感染などが生じ，時に長期間これらの症状が持続して難治性となることがある。今回われわれは，外陰部パジェット病の両鼠径部リンパ節転移症例で，リンパ節郭清後に難治性のリンパ漏を生じた1例を経験した。顕微鏡下にリンパ漏開口部のZ縫合を行い，さらにリンパ囊胞の被膜をflap状に挙上してZ縫合した部分を被覆した。これらによりリンパ漏の閉鎖を行うことができたので，文献的考察を加え報告する。

　Inguinal lymphatic leakage after an inguinal lymph node dissection is a potentially intractable complication. Several treatment modalities for such leakage have been reported, and we describe a simple and less invasive new technique for the treatment of lymphatic leakage. A 77-year-old male had a left inguinal lymphocele after undergoing a bilateral inguinal lymph node dissection for lymph-node metastasis of Paget’s disease. After the microsurgical ligation of three lymphatic fistulas under a light microscope, the area of the fistulas was covered with a capsule flap of lymphocele. The patient’s postoperative course was uneventful, with no recurrence of lymphatic leakage.
