フルシクロビンPETの読影法と課題

Image interpretation and challenges of 18F-fluciclovine PET

合成アミノ酸であるフルシクロビンを用いたPET検査が2024年7月に保険診療として承認されてから1年以上が経過した。フルシクロビンPET画像の読影に際して，本検査の臨床的位置づけと可視化されている集積の意義を十分に理解したうえで行えば，MRIでは得られない情報を得ることができる。特に最近は，PET装置の進歩によりPETの感度や分解能が上がり，淡い集積についても詳細に評価できるようになってきている。本稿では，フルシクロビンPETによる集積の強度で分けて，それぞれの集積強度での読影の注意点を概説する。

Fluciclovine PET became available in routine clinical practice in 2024 and has since been utilized for the preoperative evaluation of gliomas. While this imaging modality serves a complementary role to preoperative MRI, it is essential to interpret the findings with a thorough understanding of its characteristics. In this article, we classify fluciclovine uptake into four patterns based on intensity―no uptake, low uptake, moderate uptake, and high uptake―and provide an overview of the key points and considerations for evaluating each pattern. I hope this article serves as a useful guide for clinicians involved in image interpretation.
