Pulmonary hypertension(PH)complicating interstitial lung disease (ILD) is recognized as a condition with poor prognosis;nevertheless, advances in therapy have heightened the importance of early detection and timely intervention. Although right heart catheterization remains the diagnostic gold standard, noninvasive imaging with CT and MRI enables screening and pathophysiologic assessment of PH. Cardiac magnetic resonance(CMR)plays a central role by providing highly reproducible and accurate evaluation of right-ventricular morphology and function. Moreover, combining CMR with the increasingly applied 4D flow MRI offers the potential for a comprehensive appraisal of pulmonary arterial hemodynamics. This article outlines guideline-based PH classification, summarizes contemporary imaging strategies, and discusses future perspectives in the evaluation of PH-ILD.
