間質性肺炎に合併する肺癌の画像的特徴

慢性間質性肺炎は一般に緩徐に進行することが多く，画像で経過観察されることが多い。慢性間質性肺炎の合併症として，間質性肺炎の急性増悪，肺癌，気胸，縦隔気腫，感染症などが挙げられる。急性増悪は急速に進行する呼吸困難で発症し，画像上は両肺に広範な陰影が認められ，病理学的にびまん性肺胞障害が認められ，予後不良である。一方，慢性間質性肺炎では肺癌のリスクが高く，こちらも予後不良とされている。本稿では，慢性間質性肺炎に合併する肺癌について画像所見の特徴を中心に解説する。

Chronic interstitial pneumonia, particularly idiopathic pulmonary fibrosis（IPF）, progresses slowly but is frequently complicated by acute exacerbations and lung cancer. Lung cancer develops in 10～30％ of IPF patients, mainly elderly male smokers, and often arises at fibrotic-normal lung interfaces or within fibrotic areas. Subtle abnormalities, including tiny lesions, irregular small opacities, or pneumonia-like changes, should prompt careful follow-up through comparison with previous imaging studies. Prognosis remains poor, and invasive procedures carry significant risks of acute exacerbation.
