限局型小細胞肺癌に対するinvolved-field radiation therapyによる化学放射線療法

Involved-field radiation therapy for patients with limited disease small cell lung carcinoma：a single-institution experience

限局型小細胞肺癌（limited disease small cell lung carcinoma：LD-SCLC）に対する根治的化学放射線療法は，従来は予防的リンパ節領域照射（elective nodal irradiation：ENI）による治療が広く行われていた^1）が，2020年のASTROガイドラインではFDG-PET評価を行ったうえでの病巣部照射（involved-field radiation therapy：IFRT）が推奨されている^2）。当院では有害事象軽減の目的からCT baseの治療計画を導入した2000年以降，肺癌の根治的放射線治療を原則IFRTにて行っている。今回我々は，LD-SCLCに対するIFRTを用いた化学放射線療法の治療成績を後ろ向きに評価し，過去の治療報告との比較を行い安全性，有効性を検討した。

In our hospital, radical radiotherapy for lung cancer is generally performed by involved-field radiation therapy（IFRT）to reduce adverse events. In this article, we retrospectively evaluated the outcomes of IFRT-based chemoradiotherapy for limited disease small cell lung cancer, and compared the safety and efficacy with those reported in previous studies. The results showed that the lymph node recurrence rate and survival rate after IFRT in our institution were comparable to those reported in the past using ENI（elective nodal irradiation：prophylactic lymph node irradiation）, while esophagitis of G3 or higher was reduced.
