子宮腺肉腫の1例

A case of uterine adenosarcoma

子宮腺肉腫は，ClementとScullyによって報告されたミュラー管由来の良性上皮成分と低悪性度の肉腫成分からなるまれな上皮性・間葉性混合腫瘍である^1）。腺肉腫には低悪性度のものと，一部に上皮成分を伴わない高悪性度の肉腫成分の増殖がみられるものがあり，これが腫瘍の25％以上を占める場合には肉腫成分の過剰増殖（sarcomatous overgrowth：SO）を伴う腺肉腫と呼ばれる^2）。子宮腺肉腫の画像所見についてまとまった報告はなく，拡散強調像やdynamic studyを含めたMRI所見の報告は限られている。今回我々は，SOを伴わない子宮腺肉腫の1例のMRI所見について報告する。

We report MRI findings of a case of uterine adenosarcoma in a woman in her 50s with abnormal uterine bleeding for a year. T2-weighted images showed a well-defined solid mass with multiple small cysts in the uterine cavity. Diffusion-weighted images showed low signal intensity of the mass, and the mean apparent diffusion coefficient value in the mass was 1.5×10^－3 mm²/s. Dynamic contrast-enhanced MR images showed a relatively weak and gradual enhancement of the mass compared to the uterine myometrium. Surgery was performed, and adenosarcoma without sarcomatous overgrowth was confirmed pathologically.
