多発肺転移との鑑別が問題となった肺ランゲルハンス組織球症の1例

A case of pulmonary Langerhans histiocytosis that mimicked multiple pulmonary metastases

初期の肺ランゲルハンス組織球症（pulmonary Langerhans cell histiocytosis：PLCH）は，結節性病変を主体とした陰影を示す。そのため，担癌患者において多発肺転移との鑑別が問題となることがある。進行したPLCHで認められる嚢胞性病変は特徴的ではあるが，肺転移も空洞形成をきたすことがあるため転移との鑑別が問題となる場合がある。今回，セミノーマの術後に生じ，多発肺転移との鑑別のために，生検によって診断を確定したPLCHの1例を経験したため，文献的考察を加えて報告する。

Early pulmonary Langerhans cell histiocytosis（PLCH）manifests primarily as nodular lesions. Cystic changes may be diagnostic as the disease progresses, but metastatic lung tumors can also cause cystic changes. Therefore, imaging diagnosis of early PLCH in oncologic patients can be challenging as the image findings may resemble those of multiple lung metastases. Here, we report a case of PLCH that was difficult to distinguish from multiple lung metastases, along with a discussion of the literature.
