原発性肝細胞癌（以下，肝癌）の破裂による腹腔内出血は，日常臨床でしばしば遭遇することがある。しかし，肝癌の腹膜播種が破裂をきたした症例の報告は少ない。今回，肝癌の腹膜播種破裂による腹腔内出血の症例を経験したため，若干の文献的考察を加えて報告する。

The case is 54-year-old man with hepatocellular carcinoma（HCC）. He died of intraperitoneal bleeding because peritoneal dissemination of HCC ruptured. Peritoneal dissemination of HCC is uncommon, and there are few reports about ruptured dissemination in Japan. Physical factor is suggested as one of the risks of disseminated rupture, but detail is unknown. It may be a cause of death, it is important to consider the possibility of rupture of peritoneal dissemination as well as primary HCC and to take measures to prevent peritoneal dissemination rupture.
