肺多形癌（肉腫様肺癌）の2例―CT画像所見の検討

Two cases of the pleomorphic（sarcomatoid）lung carcinoma with literature review

肺多形癌（pleomorphic lung carcinoma：PLC）は肉腫様の紡錘細胞や巨細胞成分を10％以上含む低分化非小細胞癌に分類される比較的まれな上皮・悪性間葉細胞の混合性肺腫瘍である^1）2）。急速進行性で予後不良とされるがその画像や臨床像は不明な点が多い。

Radiological images of 2 pleomorphic（sarcomatoid）lung carcinoma cases were reviewed. PLC was defined as a poorly-differentiated non-small cell carcinoma containing spindle cells and/or giant cells. They showed relatively well-defined tumor with necrosis and cystic degeneration. On CT, one tumor demonstrated an intra-bronchial polypoid portion suggesting end- or intra-bronchial growth. Another one presented with slightly lobulated huge tumor with central necrosis. Pleomorphic cancer is rare, but in elderly smoker-men who present with rapid growth or huge peripheral mass with/without intra-bronchial portion would be considered in the differential diagnosis for expecting better prognosis.
